Late Package Claim Form

In order to receive a refund on shipping for a package that was delivered late, you must fill out this form completely. Please carefully read over the Late Package Claim Guidelines before submitting your claim. Failing to follow the guidelines may result in your claim being denied.

Please note: if your package is more than five days late and has not been delivered, click here.

Late Package Claim Guidelines:

In order to file a claim for a refund on the cost of your shipping, you must wait to file until after the package has arrived.
Once your package arrives, you have seven days to submit a Late Package Claim.
DO NOT initiate a claim with the shipping carrier yourself as this will disqualify you from receiving a shipping refund.
If you received FREE shipping with your order, you do not qualify for a refund on shipping nor a discount code.
