Missing Package Claim Form

In order to receive a refund or replacement for a package that was never delivered, you must fill out this form completely. Please carefully read over the Missing Package Claim Guidelines before submitting your claim. Failing to follow the guidelines may result in your claim being denied.

Missing Package Claim Guidelines:

If five days have passed since your estimated or scheduled delivery date, you may submit a Missing Package Claim.
You have 21 days from the scheduled delivery date to file a Missing Package Claim.
You must not initiate a claim with the shipping carrier yourself as this will disqualify you from receiving a refund or replacement of any missing items.
If you received a package, but the contents are missing, you must fill out a Missing Items Claim Form.
