Damaged Item Claim Form

In order to receive a refund  or replacement for a damaged item(s), you must fill out this form completely. Please carefully read over the Damaged Item Claim Steps before submitting your claim. Failing to follow the steps may result in your claim being denied.

Damaged Item Claim Steps:

Within 48 hours* of delivery of your package containing damaged item(s), you must take the following steps:
    1. Take a photo or photos that clearly shows the extent of the damage
    2. Fill out and submit this form (be sure to upload the photo evidence)
    3. Keep all original packaging and the damaged item until further notice

Once your claim is submitted, we will follow-up and provide instructions on what to do with the packaging and damaged item (you may be asked to mail these items back to us or take them to your local Post Office for inspection to complete your claim).

DO NOT initiate a claim with the shipping carrier yourself as this will disqualify you from receiving a refund or replacement of the missing items(s).

Damaged items not covered by insurance will be compensated at the discretion of Sunny Smart Supplements.
