Missing/Incorrect Items Claim Form

In order to receive a refund or replacement for missing or incorrect items, you must fill out this form completely. Please carefully read over the Missing Items Claim Guidelines before submitting your claim. Failing to follow the guidelines may result in your claim being denied.

Missing Items Claim Guidelines:

Within 48 hours* of delivery, if your package is missing an item(s), you must take the following steps:
  1. Take a photo of the empty package, or a photo of the package alongside the items that did arrive
  2. Fill out and submit this form (be sure to upload the photo evidence)
  3. If the package was entirely empty, keep the package in case we need you to mail it back to us (we will provide a new shipping label)
Once your claim is submitted, we will contact you within two business days to work with you on whether to refund or replace your missing items.
The extent to which we can help will depend on whether you opted for insurance.
You must not initiate a claim with the shipping carrier yourself as this will disqualify you from receiving a refund or replacement of missing item(s).
