Missing/Incorrect Items Claim Form
In order to receive a refund or replacement for missing or incorrect items, you must fill out this form completely. Please carefully read over the Missing Items Claim Guidelines before submitting your claim. Failing to follow the guidelines may result in your claim being denied.
Missing Items Claim Guidelines:
Within 48 hours* of delivery, if your package is missing an item(s), you must take the following steps:
- Take a photo of the empty package, or a photo of the package alongside the items that did arrive
- Fill out and submit this form (be sure to upload the photo evidence)
- If the package was entirely empty, keep the package in case we need you to mail it back to us (we will provide a new shipping label)
